Why Low Testosterone and Anemia Often Show Up Together

A man in his fifties walks in exhausted, and his blood panel flags two things at once: low testosterone and anemia. That pairing turns up far more often than most men expect. Testosterone is one of the signals that tells bone marrow to build red blood cells, so when it falls, hemoglobin tends to drift down with it. The catch is that anemia has plenty of other causes, a few of them serious, and hormones should never be the first thing you blame.

What Low Testosterone and Anemia Actually Have in Common

Anemia in men is generally defined as a hemoglobin below about 13 g/dL. It becomes steadily more common past 60, and in a frustrating share of cases the standard workup comes back clean: no bleeding, no iron deficiency, no kidney disease. Clinicians call that unexplained anemia of aging, and for decades it was filed under things that just happen to older men.

Testosterone reframed that. Levels drop roughly 1% a year after 30, and the men who end up anemic skew heavily toward the bottom of the range. A large analysis of older adults in Archives of Internal Medicine found that men with low testosterone were both more likely to be anemic at the outset and more likely to become anemic over the next three years. That is association, not proof. But it pointed researchers somewhere useful.

How Testosterone Talks to Your Bone Marrow

Three mechanisms do most of the work. Testosterone prompts the kidneys to release more erythropoietin, the hormone that places the order for new red cells. It suppresses hepcidin, the liver peptide that acts as a gatekeeper on iron, which means more iron absorbed from food and more released from storage – exactly what marrow needs to build hemoglobin. And it appears to reset the body’s erythropoietin-to-hemoglobin set point, so a given hormone level now supports a higher red cell mass.

Researchers measured the hepcidin effect directly, and it was dose-dependent: more testosterone, less hepcidin, more iron reaching the marrow. It is the same lever that pushes hematocrit too high in some men on therapy. One mechanism, two directions – which is also why low testosterone and anemia so often travel together on the same lab report.

The Trial That Put Low Testosterone and Anemia on the Same Page

The Testosterone Trials enrolled 788 men aged 65 and over with clearly low levels. Of those, 126 were anemic at the start; 64 had an identifiable cause, and 62 fell into the unexplained bucket. After a year on testosterone gel, 54% of the unexplained group saw hemoglobin climb by at least 1.0 g/dL, compared with 15% on placebo. The anemia resolved outright in 58.3% of treated men versus 22.2% of controls. Men whose anemia had a known cause improved as well, just less dramatically.

One gram per decilitre reads as modest on a lab report. In a 72-year-old it is often the gap between arriving at the top of the stairs winded and not thinking about the stairs at all.

The Labs Worth Running Before You Blame Testosterone

Low testosterone and anemia can coexist without one causing the other, and treating the hormone will not fix a bleeding ulcer. Before anyone connects the two, a proper blood panel should rule out the usual suspects:

  • Ferritin and full iron studies. Iron deficiency is the single most common cause worldwide, and in a man over 50 it demands a look at the gut before anything else.
  • B12 and folate. Cheap tests, easily missed, and both produce fatigue that mimics the classic symptoms of low testosterone.
  • Creatinine and eGFR. Failing kidneys make less erythropoietin, which produces anemia through the same pathway from the other end.
  • MCV and a hemoglobin electrophoresis where the numbers warrant it. Thalassemia trait is common across Southeast Asia, including Indonesia, and a small red cell volume with normal iron is the tell. We see this often enough at the clinic that it is worth naming.
  • Reticulocyte count and CRP. These separate a marrow that is not producing from one whose output is being destroyed or suppressed by inflammation.

Run the testosterone properly too. A single morning total reading is a starting point, not a diagnosis, which is why free versus total testosterone matters and why timing and repeat testing change the answer more than men realise. Metabolic health muddies things further, since insulin resistance drags testosterone down and chronic inflammation drags hemoglobin down alongside it.

When Treating Low Testosterone Helps, and When It Overshoots

For a man whose workup leaves low testosterone and anemia as the only two findings, testosterone replacement therapy is a reasonable answer to both problems at once. Hemoglobin typically responds over three to six months and plateaus by twelve.

Push the same lever hard enough and it goes the other way. Erythrocytosis is the most common lab abnormality on testosterone therapy, and most guidelines treat a hematocrit above 54% as the point to intervene rather than watch. That usually means trimming the dose, moving to smaller and more frequent injections to flatten the peaks, and occasionally donating blood. Anyone on therapy needs hematocrit checked at three months, six months, and yearly after that – the same blood count that flagged the anemia now watches for the opposite problem.
