Option 1: Try to Conceive While on TRT

Although TRT reduces sperm production in most men, some remain fertile enough to conceive. In fact, many men on TRT have fathered children without additional treatment.

However, this isn’t guaranteed. Sperm count can drop over time, especially with long-term TRT. If you’re hoping to get pregnant while continuing TRT, you’ll be taking a chance.

Also important: TRT is not a form of birth control. Even with a low sperm count, pregnancy is still possible—so use protection if you’re not planning for a child.

We generally don’t recommend this option unless you’re actively trying to conceive soon and fully understand the risks.

Option 2: Freeze Your Sperm Before Starting TRT

Sperm freezing is one of the safest ways to preserve your future fertility. This involves providing a sample before beginning TRT and storing it in a sperm bank for later use.

Even if you’re not ready to become a parent now, it gives you the freedom to make that decision later without worrying about whether TRT has affected your fertility.

Cost can be a concern, but for many men, it’s a worthwhile investment.

Option 3: Use hCG and/or hMG While on TRT

If you want to stay on testosterone and remain fertile, this option may be ideal. Human chorionic gonadotropin (hCG) mimics the body’s luteinizing hormone, which stimulates the testicles to keep producing testosterone and sperm.

hMG (human menopausal gonadotropin) can also be added to mimic FSH (follicle-stimulating hormone), which further supports sperm development.

Studies show that using hCG alone—or combined with hMG—can preserve fertility in men on TRT. In one study, 26 men maintained sperm production for over a year while using hCG alongside testosterone therapy.
